READ before ordering:
Layouts will be done in the order of the comments.
Your form will be delete once complete and payed.
No stealing,editing,using parts of my codes or claiming as yours.
Don't use the layout if I didn't make it for you.
I'll report you if you don't follow these rule. >:[
If you don't like the layout,you'll have to pay half price for it. I will make some minor changes if desired.If you order a layout,you accept these rules.
